7 AAC 05.931 - Disclosure to immunization registries
(a) The state
registrar shall disclose data collected from certificates of birth or filings
under 7 AAC 05.705 to enroll each child
born in this state in any immunization registry maintained by the
department.
(b) The state registrar
shall submit an electronic data file to any immunization registry maintained by
the department within seven days of the information being received by the
department. Information to be submitted to any immunization registry maintained
by the department must include, if available,
(1) the name of the child;
(2) the date of birth of the child;
(3) the hospital or other institution of
birth of the child;
(4) the
certificate of birth number for the child;
(5) the name, date of birth, and residence
address of the mother of the child; and
(6) the name and date of birth of the father
of the child.
(c) In
this section, "immunization information system" has the meaning given in
7 AAC 27.900.
