Ariz. Admin. Code § R19-2-516 - Complaints Pertaining to Pari-mutuel Operations
A. When a patron makes a complaint regarding
the pari-mutuel department to a permittee, the permittee shall immediately
issue a complaint report setting out:
1. The
name of the complainant;
2. The
nature of the complaint;
3. The
name of the persons, if any, against whom the complaint was made;
4. The date of the complaint;
5. The action taken or proposed to be taken,
if any, by the permittee.
B. The permittee shall submit every complaint
report to the Department within 48 hours after the complaint was
made.
