Ariz. Admin. Code § R4-26-414 - Complaints and Investigations
A.
Anyone, including the Board, may file a complaint. A complainant shall ensure
that a complaint filed with the Board involves:
1. An individual licensed under this Article;
or
2. An individual, including an
applicant, believed to be engaged in the unlicensed practice of behavior
analysis.
B. Complaint
requirements. A complainant shall:
1. Submit
the complaint to the Board in writing; and
2. Provide the following information:
a. Name and business address of licensee or
other individual who is the subject of complaint;
b. Name and address of complainant;
c. Allegations constituting unprofessional
conduct;
d. Details of the
complaint with pertinent dates and activities;
e. Whether the complainant has contacted any
other organization regarding the complaint; and
f. Whether the complainant has contacted the
licensee or other individual concerning the complaint and if so, the response,
if any.
