Cal. Code Regs. Tit. 10, § 3662 - Accreditation Application
(a) Course providers desiring to be
accredited and/or to have a course accredited shall submit Course Provider
Accreditation, REA Form 3013 (Rev. 04/01/2020), Course Accreditation and
Description, REA Form 3014 (Rev. 04/01/2020) and BREA Topic Matrix, REA Form
3015 (Rev. 5/8/00), which are herein incorporated by reference, as
applicable.
(b) The application
shall be signed by the course provider's Chief Executive Officer.
(c) The Chief Executive Officer of the course
provider may delegate authority to sign the Course Provider Accreditation, REA
Form 3013 (Rev. 04/01/2020), Course Accreditation and Description, REA Form
3014 (Rev. 04/01/2020) and BREA Topic Matrix, REA Form 3015 (Rev. 5/8/00), by
filing with BREA a written delegation of authority to act on behalf of the
course provider for accreditation matters.
(d) Only one such delegation of authority
shall be valid at any point in time for the education activities of the course
provider within a particular state.
(e) The delegation of authority shall be
valid for a maximum of four years, and may be revoked by the Chief Executive
Officer by filing a written revocation of the delegation of authority with
BREA. A new delegation of authority, in writing, must be submitted to, and
received by, BREA within 30 days of a change in the course provider's Chief
Executive Officer.
