Cal. Code Regs. Tit. 16, § 4222 - Duration and Renewal of License
(a) The initial license fee shall be prorated
from the month of issuance to the month in which the second birthday of the
licensee occurs after the issuance of the license.
(b) A naturopathic doctor's license shall
expire at 12 midnight on the last day of the birth month of the licensee during
the second year of a two-year term if not renewed.
(c) To renew an unexpired license, a licensee
shall, before the time at which the license would otherwise expire, apply for
renewal and pay the renewal fee required in Section
4240.
(d) The renewal application shall include all
of the following:
(1) A statement by the
licensee specifying whether the licensee was convicted of a crime or
disciplined by another public agency during the preceding renewal
period;
(2) A statement by the
licensee that they have complied with the continuing education requirements
established pursuant to Section
3635 of the Code, as applicable.
The licensee shall attach documentation to the renewal application verifying
the successful completion of the required continuing education.
(3) A statement by the licensee that their
representations on the renewal form are true, correct, and contain no material
omissions of fact, signed under penalty of perjury.
