Cal. Code Regs. Tit. 17, § 30408 - Certificate and Permit Fees
(a) For
any certificate or permit issued pursuant to this subchapter in accordance with the
Radiologic Technology Act (Health and Safety Code section
27(f)),
the application fee shall be $127.00 for licentiates and $112.00 for technologists
and limited permittees. If required to pass an examination to obtain the certificate
or permit, the examination fee shall be $112.00 for each examination administered by
the Department or as specified by the entities or organizations designated by the
Department to administer Department-approved examinations.
(b) The fee for repeating an examination failed
within the previous 12 months shall be $112.00 per examination.
(c) Each individual applying to renew a
certificate or permit shall pay an annual renewal fee of $52.00. The renewal fee
shall be collected biennially and such fee shall be twice the annual renewal
fee.
(d) The fee for a duplicate
certificate or permit shall be $2.00.
(e) The penalty fee for renewal of any expired
certificate or permit shall be $8.00 and shall be in addition to the fee for
renewal.
(f) Failure to pay the annual
fee for renewal on or before the expiration date of the certificate or permit shall
automatically suspend the certificate or permit. If the annual renewal fee is not
paid within six months following such date, the certificate or permit shall be
revoked. A certificate or permit revoked for nonpayment of the renewal fee may be
reinstated within five years from the time of revocation upon payment of the penalty
fee specified in subsection (e) plus twice the annual renewal fee specified in
subsection (c). If the application for reinstatement is not made within five years
from the date of suspension of the certificate or permit, the certificate or permit
shall be canceled and shall not be subject to reinstatement.
(g) Fees required by this section are:
(1) Subject to adjustment pursuant to section
100425
of the Health and Safety Code; and
(2)
Nonrefundable.
