Cal. Code Regs. Tit. 22, § 78339 - Basic Program Service: Social Services

(a) The adult day health center shall provide social services to assist participants and their families to solve medical social problems that arise as a result of the participant's disability.
(b) An unlicensed social worker shall only provide those social work services for which no license is required under Business and Professions Code section 4996.
(c) Medical social services shall consist of at least the following:
(1) Initial and continuing assessment of the participant by the medical social worker in the formation of an individual plan of care by the multidisciplinary team.
(2) Maintaining signed progress records in the participant's health record at least quarterly.
(3) Assuming responsibility for implementation and coordination of the discharge plan.
(4) Providing referral to outside resources and assisting with counseling when applicable.
(5) Providing problem oriented discussion groups among participants.
(6) Serving as a liaison with the participant's family and home.
(7) Providing referral and follow-up for referred persons not appropriate for adult day health care.
(d) Medical social services shall be provided or supervised by a social worker.
(e) The medical social worker shall be a full-time employee of the adult day health center.
(f) A social work assistant or social work aide shall provide medical social services only under the supervision of the medical social worker.
(g) Space to allow privacy shall be provided for the medical social worker when counseling participants.
