Cal. Code Regs. Tit. 22, § 84268.4 - Removal and/or Discharge Procedures
(a) The licensee or designee shall cooperate
with the placement agency in developing a discharge plan for the
child.
(b) The following items
shall accompany the child to the new placement:
(1) Copies of case files, assessments, the
discharge plan, the daily activity schedule, and the child's medical
history.
(2) The child's prognosis
and the facility's recommendations for further treatment, education, and
placement.
(A) The discharge plan may include
this information.
(3)
Personal clothing, toys and objects and the child's
scrapbook.
