Cal. Code Regs. Tit. 22, § 97046.3 - Form of Authentication
(a) Each annual
consolidated financial report as specified by Section
128734.1
of the Health and Safety Code shall be accompanied by a report certification
signed by a duly authorized official that certifies under penalty of perjury
that, as applicable, to the best of the official's knowledge and information,
each statement and amount in the accompanying report is believed to be true and
correct.
(b) Organizations shall
submit report certifications through the Department's website using
SIERA.
(c) Report certifications
signed by health facility consultants are not acceptable.
