Cal. Code Regs. Tit. 22, § 97177.70 - Hospital Data Contact Person, User Account Administrator
(a) Each hospital at which CABG surgeries are
performed shall designate a primary CCORP data contact person. A hospital shall
notify CCORP of the designation in writing, by electronic mail or through the
Cardiac Online Reporting for California (CORC) system within 30 days of the
effective date of this regulation or within 30 days of beginning or resuming
operation. A notification shall include the designated person's name, title,
telephone number(s), mailing address, and electronic mail address.
(b) A hospital shall notify CCORP in writing,
by electronic mail or through the CORC system within 30 days after any change
in the person designated as the primary CCORP data contact person, or in the
title, telephone number(s), mailing address, or electronic mail address, of the
individual.
(c) Each hospital shall
designate up to three User Account Administrators pursuant to Subsection (I) of
Section 97170. For each User Account
Administrator there must be a signed CORC User Account Administrator Agreement
Form (OSH-CCORP 757 (Rev. 06/17)) and hereby incorporated by reference,
submitted to the Office via fax, email, or upload through the Cardiac Online
Reporting for California (CORC) system. Each hospital shall notify CCORP in
writing, by electronic mail or through the CORC system within 30 days after any
change in a designated User Account Administrator's name, title, telephone
number(s), mailing address, or electronic mail address.
(d) Each hospital is responsible for
submitting its own online data report to CCORP. The hospital shall be
responsible for ensuring compliance with regulations and reporting requirements
when a third party vendor assists a hospital with CCORP data.
