Conn. Agencies Regs. § 16-304-D4 - Forms

Certificates of insurance shall be filed on Form E, entitled, "Uniform Motor Carrier Bodily Injury and Property Damage Liability Certificate of Insurance," prescribed by the public utilities commission and the National Association of Regulatory Utility Commissioners. This certificate shall cover all vehicles operated by the carrier within the state of Connecticut and shall be signed by a person designated to the public utilities commision, by the insuring, bonding or surety company offering the insurance, as the agent authorized to sign such certificates of insurance, and shall certify to public utilities commission that such policy or policies of insurance in at least the minimum amounts hereinafter prescribed have been issued to the motor carrier of property for hire, and that such policy or policies apply with respect to the ownership, maintenance or use.
