Fla. Admin. Code Ann. R. 64B3-11.003 - Requirements for Continuing Education Programs
Programs seeking Board approval shall meet the following requirements:
(1) All educational
courses conducted within the program shall meet the standards for continuing
education courses as outlined in Rule
64B3-11.002, F.A.C.
(2) Programs shall receive a program number
upon approval and shall use this number on all correspondence with the Board
and the Department.
(3) Providers
shall initially designate and subsequently update as appropriate a person to
assume responsibility for continuing education courses for clinical laboratory
personnel.
(4) A system of record
keeping shall be maintained which provides for storage of individual course
information for a period of at least 3 years.
(5) Within 30 days of the continuing
education program, each participant shall be provided with a certificate or
letter of attendance which shall include the participant's name, license
number, course title, number of continuing education hours earned by specialty
area, dates of attendance, program provider's name, approval number, and the
signature of the provider.
