Fla. Admin. Code Ann. R. 64I-4.005 - Smoking or Vaping Cessation Program Approval

If a smoking or vaping cessation program is conducted within an enclosed indoor workplace and requires smoking or vaping during its sessions, the department must approve the program in order to permit smoking or vaping within an enclosed indoor workplace. Programs seeking such approval must submit a completed Department of Health Application for Smoking or Vaping Cessation Programs, DH9000-CHP, effective 08/2019, incorporated by reference and available at http://www.flrules.org/Gateway/reference.asp?No=Ref-11870, or by email at FCIAA@flhealth.gov.
