Ga. Comp. R. & Regs. R. 120-2-2-.20 - Initiation of Adjudication
1. An adjudication is initiated when a party
files a petition.
2. The petition
may be sent to the Department by either:
a.
Mail: ATTN: Administrative Procedure Division,
Department of Insurance
2 Martin Luther King Jr. Dr.
716 West Tower, Floyd Bldg.
Atlanta, GA 30334, or
b. Email: AdminProc@oci.ga.gov
3. The petition must contain the
following:
a. addressed to the
Commissioner;
b. signed by the
petitioner or his/her counsel;
c. a
brief statement of the nature of the proceeding,
d. the identity of known parties,
e. the jurisdiction under which the
adjudication is initiated,
f. a
brief statement of the general allegations of fact and the issues to be
adjudicated,
g. the legal authority
that constitutes a basis for the adjudication, and
h. a brief statement of the nature of the
relief sought.
