Ga. Comp. R. & Regs. R. 120-2-49-.15 - Notification Required
(1) Where the services of an administrator
are utilized, the administrator shall provide a written notice approved by the
insurer, or self-insurer, to insured or self-insured individuals, advising them
of the identity of and relationship among the administrator, the group
policyholder and the insurer or self-insurer. Where an administrator collects
funds, it must identify and state separately in writing to the person paying to
the administrator any charge or premium for insurance coverage or self-funded
benefits, the amount of any such charge or premium specified by the insurer or
self-insurer for such insurance coverage or self-funded benefits.
(2) Each administrator shall identify to the
Commissioner any ownership interest or affiliation of any kind with any
insurance company responsible for providing benefits directly or through
reinsurance to any plan for which the administrator provides administrative
services.
