Haw. Code R. § 11-99-81 - Social work services
(a) Social work
services shall be provided by the facility and available to all residents,
their families, and other significant persons to enable them to deal with the
impact of mental retardation on individual and family functioning.
(b) There shall be sufficient, appropriately
qualified staff to meet the social service needs of all residents admitted to
the facility. The ratio of social work personnel to residents shall be:
(1) Less than fifty residents there shall be
a minimum eight hours consultation per month from a qualified social
worker.
(2) For fifty to
ninety-nine residents there shall be a minimum of one half-time qualified
social worker.
(3) For every
hundred residents there shall be a minimum of one full time qualified social
worker.
(c) Social work
services shall be documented in each resident's record and include at least:
(1) An initial social history and assessment
of current social and emotional needs.
(2) A current social work plan to meet
identified needs.
(3) Regular
progress notes indicating the resident's status.
(4) Appropriate discharge plans.
(5) Evidence of regular and at least annual
review of social work and discharge plans in conjunction with the overall plan
of care.
(d) Social work
staff shall have appropriately furnished facilities which are easily accessible
to the residents being served and which provide privacy for interviews,
counseling, and telephone conversations.
