Haw. Code R. § 16-77-51 - Restoration of forfeited license

Forfeited licenses may be restored by filing a renewal application along with all required documents, and paying the renewal fees, including the recovery and education fund fees, plus a penalty fee on or before November 30 of the renewal year. Licensees who fail to restore their licenses by November 30 shall be required to apply as new applicants, and shall file a new license application form along with all required supporting documents and fees.
