Haw. Code R. § 16-83-8 - Record keeping
The hearing aid dealer and fitter shall retain for five years after the dispensing of a hearing aid a copy of any written statement from a physician required under section 16-83-4 or waivers as required under sections 451A-5(9) and 451A-14.1, HRS. The book, file, or microfilm of the authorizations and statements shall at all times be open to inspection by the board of hearing aid dealers and fitters and other law enforcement agencies.
