Haw. Code R. § 16-85-2 - Notification and filing of names, addresses, and changes

Any person holding a license or certificate shall file the person's mailing address with the board and shall immediately notify the board in writing of any and all changes which occur within thirty days of the change. Any requirements that the board provide notice to licensees and certificate holders shall be deemed met if notice is sent to the address on file with the board.
