Haw. Code R. § 16-85-37 - Certification of compliance

At the time of renewal, each licensee shall certify on a form provided by the board, that the licensee has complied with the CME requirements as set forth herein. The certification shall be under oath if required by the board. The board may require any licensee to submit, in addition to the certification, further evidence satisfactory to the board demonstrating compliance with the CME requirements set forth herein.
