Ill. Admin. Code tit. 56, § 350.410 - Reporting Fatalities, Hospitalizations, Amputations and Loss of Eye Incidents to the Illinois Department of Labor
a) Basic Requirements
1) All public sector employers must report:
A) All work-related fatalities within 8
hours; and
B) All work-related
inpatient hospitalizations, all amputations, and all losses of an eye within 24
hours.
2) Employers must
orally report by calling Illinois OSHA's 24/7 confidential number at
(217)-782-7860.

Implementation
1) The reporter must give the
following information for each fatality, hospitalization, amputation, or loss
of an eye incident:
A) The establishment
name;
B) The location of the
incident;
C) The time of the
incident;
D) The number of
fatalities or hospitalized employees;
E) The names of any injured
employees;
F) The reporter's
contact person and his or her phone number; and
G) A brief description of the
incident.
2) Every
fatality or hospitalization incident resulting from a motor vehicle accident
must be reported.
3) Fatalities
caused by a heart attack at work must be reported. The Division Manager or
designee will decide whether to investigate the incident, depending on the
circumstances of the heart attack.
4) If the employer does not learn of a
reportable incident at the time it occurs and the incident would otherwise be
reportable under this Section, the employer shall make the report within 8
hours after the incident is reported to the employer or any agent or employee
of the employer.
