Ill. Admin. Code tit. 68, § 1248.90 - Establishing, Relocating or Changing Ownership
a) Any person who desires to establish,
relocate or change the ownership of a euthanasia agency shall file an
application on forms supplied by the Department, together with the fee required
in Section
1248.20.
b) Upon determination that the application is
in good order, an inspection of the premises will be conducted to determine
compliance with Section 25 of the Act.
