10 IAC 1.5-2-4 - Amended reports

Sec. 4.

(a) The holder must file an amended unclaimed property report if any change occurs after a report is submitted to the attorney general. The amended report must include a statement of the reason for the change, when appropriate, including, but not limited to, the following reasons:
(1) Change of owner's name.
(2) Change of owner's last known address.
(3) Change of date of last transaction.
(4) Change of legal description of mineral interest in land.
(5) Change of decimal percentage of ownership of mineral interest in land.
(6) Change in original amount reported.
(7) Error in the report.
(b) Amendments must be filed no more than thirty (30) days from the latter of either the date of the change or the date the holder became aware of the change.
