Kan. Admin. Regs. § 82-3-130 - Completion reports; penalty
(a) Within 120 days
of the spud date or commencement of recompletion of a well, the operator shall
file an original and two copies of an affidavit of completion with the
conservation division, except as provided by subsection (b).
(b) If the time requirement for cementing the
additional casing, pursuant to 82-3-106(c)(2)(B), is greater than 120 days, the
time for filing the affidavit of completion and two copies shall be extended
accordingly.
(c) The affidavit of
completion shall be filed regardless of the manner in which the well is
completed or recompleted, including a well that is dry and abandoned. The
affidavit of completion shall be submitted on forms furnished by the
commission. The affidavit shall be accompanied by wireline logs of the well, if
run. The failure to file the affidavit of completion within the time required
shall be punishable by a $500 penalty.
(d) Each operator shall attach legible
documentation to the affidavit of completion, showing the type, amount, and
method of cementing used on all casing strings in the wellbore. The
documentation may consist of invoices, job logs, job descriptions, or other
similar service company reports.
