10-144 C.M.R. ch. 301, § 200-FS 222-3 - Applicant/Authorized Representative
1.
Applicant - The person in whose name the application
is made and in whose name the FS coupons will be mailed.
2.
Authorized
Representative - A responsible non-household member, designated by
the head of the household, or another responsible adult member of the
household, to act on behalf of the participating household.
NOTE: With the exception of residents of drug and alcohol treatment centers, an authorized representative must be designated in writing by a responsible household member.
An authorized representative must be aware of relevant household circumstances.
WHO CANNOT BE AN AUTHORIZED REPRESENTATIVE
1. State
employees involved in the certification and/or issuance process;
2. retailers who are authorized to accept FS
coupons;
3. individuals
disqualified for an intentional program violation during their disqualification
period, unless no one else is available.
