16-163 C.M.R. ch. 5, § B-2 - Requirements for licensure
An application will not be accepted as complete unless it includes all materials required to be evaluated for licensure. To obtain a new license an applicant must
1. Have
reached their eighteenth birthday.
2. Submit tire following to Maine EMS:
1. A completed Maine EMS application signed
by the applicant. An application will not be accepted as complete unless it
includes all materials required to be evaluated for licensure.
2. Hold a valid state driver's license
pursuant to Title 29-A, chapter 11, subchapter 1. Or qualify for an exemption
under MRS 29-A Chapter 11 SS 1251(6).
3. Provide the following:
1. The state or province of issuance of the
driver's license,
2. The driver's
license number, and
3. The driver's
license expiration date.
4. A complete history of criminal convictions
as well as civil infractions for alcohol or drugs. Maine EMS will consider this
to the extent allowed by Maine Law.
5. A complete history of any action taken
against any emergency medical services certification or license or professional
certification or license that the applicant currently holds or has ever
held.
