Md. Code Regs. 10.07.10.03 - Application for a License
A. The
applicant shall file an application for a license to operate a home health
agency on forms provided by the Department.
B. The application for licensure shall
contain the name and address of each officer and director of the home health
agency. Those corporations having more than 100 owners shall name those having
an interest of 2 percent or more. Persons related by marriage or blood to the
third degree of consanguinity shall each be named if the sum of their separate
holdings exceeds 2 percent.
