205 CMR 6.23 - Patron Information

(1) Each short-term or long-term account holder shall provide such personal information as the association and the Commission require, on a form acceptable to the Commission. The following minimum information must be collected for all account holders:
(a) Full legal name;
(b) Physical address of the principal residence;
(c) Mailing address of the applicant, if different from above;
(d) Telephone number;
(e) Date of birth;
(f) Social Security Number or Federal Identification Number;
(g) Date;
(h) Signature.
(2) Each application may be made in person at the racetrack or submitted by mail, by telephone, or by other electronic media. Each application submitted over the telephone or by other electronic media will be subject to electronic verification with respect to name, physical address of principal residence, date of birth and Social Security number or Federal Identification number by either a national independent individual reference service company or other technology which meets or exceeds the reliability, security, accuracy, privacy and timeliness provided by individual reference service companies. Mailed applications must include a photocopy of the applicants drivers license or other photo ID that includes a signature.
(3) All applicants must be at least 18 years old.
