803 CMR, § 2.12 - Electronic Submission of CORI Acknowledgment Forms
(1)
A requestor may collect CORI Acknowledgment Forms through electronic means
including, but not limited to, the requestor's electronic application. When
CORI Acknowledgment Forms are collected electronically, the requestor shall
comply with all provisions set forth in
803
CMR 2.11.
(2) To satisfy the requirement pertaining to
verification of the subject's identity as set forth in
803
CMR 2.11(1)(b), the subject
may submit a notarized copy of the CORI Acknowledgment Form as provided in
803
CMR 2.11(5)(c). Such
notarized form shall be either uploaded into the requestor's electronic system
or transmitted electronically to the requestor prior to the submission of the
subject's CORI request. If the requestor is unable to verify the subject's
identity in person and unable to obtain such notarized form, then it shall
comply with the provisions of
803
CMR 2.11(5)(e) or (f).
