Mich. Admin. Code R. 408.31144 - Notification of changes
Rule 1144.
(1)
Manufacturers shall notify the bureau and the commission in writing within 10
days of any of the following occurrences:
(a)
The corporate name is changed.
(b)
The main address of the company is changed.
(c) There is a change in 25% or more of the
ownership interest of the company within a 12-month period.
(d) The change in location of any
manufacturing facility which may supply components or units for sale and use in
the state of Michigan.
(e) The
establishment of a new manufacturing facility which may supply components or
units for sale and use in the state of Michigan.
(f) There are changes in principal officers
of the firm.
(2)
Evaluation agencies and inspection agencies shall notify the bureau and the
commission in writing within 10 days of any of the following occurrences:
(a) The company name is changed.
(b) The main address of the company is
changed.
(c) There is a change in
25% or more of the ownership interest or control of the company within a
12-month period.
(d) The change in
location of any testing facility which may supply services to the bureau or
commission.
(e) The establishment
of a new testing facility which may supply services to the bureau or
commission.
(f) There are changes
in principal officers and key supervisory and responsible personnel of the
firm.
