Mich. Admin. Code R. 436.1053 - Proof of loss or destruction of a license or permit
Rule 53.
(1) To
prove the loss or destruction of a license or permit, a licensee shall make a
written statement as to the loss or destruction and shall file it with the
commission.
(2) The written
statement shall include a detailed description of the circumstances concerning
the loss or destruction of the license or permit and shall be accompanied by
any remaining part of the license.
(3) The written statement shall be signed by
the licensee.
