Mich. Admin. Code R. 436.1103 - Application for license; forms; required information
Rule 3.
(1) An
application for a license shall be made to the commission in Lansing on forms
approved by the commission.
(2) An
applicant for a license shall provide to the commission, or representatives of
the commission, all information necessary for investigation and processing of
the application.
