A. Transition
Assistance is a one (1) time set-up expense for people who transition from
institutions (ICF/IID or Title XIX nursing home) to the ID/DD Waiver. They may
move to a less restrictive community living arrangement such as a house or
apartment, with ID/DD Waiver Supports or home with family and receive ID/DD
Waiver Supports.
B. To be eligible
for transition assistance the following is necessary:
1. The person cannot have another source to
fund or attain the items or support.
2. The person must be transitioning from a
setting where these items were provided for him/her and upon leaving the
setting will no longer be provided.
3. The person must be moving to a residence
where these items are not normally furnished.
4. The person's ICF/IID or nursing facility
stay is not acute or for rehabilitative purposes.
C. Items bought using these funds are for
individual use and are to be the property of the person if he/she moves from a
residence owned or leased by a provider.
D. There is a one-time, life time maximum of
$800 per person.
