Nev. Admin. Code § 483.055 - Documentation required for change of full legal name; appearance of new full legal name
1. A person who
wishes or is required pursuant to section 5 or 6 of Senate Bill No. 215,
chapter 241, Statutes of Nevada 2017, at page 1276 or 1277, as applicable (NRS
483.375 or 483.8605 ), to change the full legal name indicated on his or her
driver's license, motorcycle driver's license or identification card must
submit proof satisfactory to the Department that his or her full legal name was
changed. The proof may include, without limitation, one of the following
documents:
(a) Any document or combination of
documents required pursuant to section 5 or 6 of Senate Bill No. 215, chapter
241, Statutes of Nevada 2017, at page 1276 or 1277, as applicable (NRS 483.375
or 483.8605 );
(b) A certificate of
domestic partnership issued by the Secretary of State of this State or any
authorized agency of another state; or
(c) Any other document that the Department
determines is acceptable as proof of the change of the person's full legal
name.
2. The Department
shall indicate the full legal name of the person requesting the change of his
or her full legal name on a new driver's license, motorcycle driver's license
or identification card in a manner that is identical to the manner in which the
full legal name appears on the document or documents submitted by the person as
proof of the change of the person's full legal name.
