Nev. Admin. Code § 616B.018 - Notice to Administrator of accident or exposure to disease-causing agent or fatality from accident or exposure
1. Within 30 days
after an insurer receives notice that an employee has been:
(a) Involved in an accident; or
(b) In close proximity to or has had contact
with a disease-causing agent that may have a harmful effect on the employee,
the insurer shall notify the Administrator if the accident resulted in injury to, or the exposure to the disease-causing agent affected or is expected to affect, two or more persons.
2. Within 48 hours after the insurer receives
notice of a fatality that resulted from:
(a)
An accident that an employee was involved in; or
(b) The close proximity to or contact with a
disease-causing agent by the employee,
the insurer shall notify the Administrator of the fatality by submitting Form D-21, Fatality Report, to the Administrator.
