N.J. Admin. Code § 7:18-1.6 - Program information; notices; submittals
(a) Unless
otherwise specified, any questions concerning the requirements of this chapter
should be directed to the Department's Office of Quality Assurance at (609)

(b) Unless otherwise specified, any
submittals of PT sample results, submittals of documents, notices, or other
communications required to be made to the Department under this chapter shall
be made to the address specified in (a) above. Applications for certification
and for renewals and modifications of certifications shall be submitted to the
address specified in (a) above.
