Ohio Admin. Code 4141-21-01 - Application for reconsideration of employer's liability determination

An employer applying for reconsideration pursuant to division (D) and/or (E) of section 4141.26 of the Revised Code shall file an application within thirty days after the determination was sent to the employer or the employer's authorized representative. Such application shall provide the name and address and official position of the person filing the application, ; the name, address, and state unemployment tax account identification number of the employer, ; and a brief statement of the reasons reason(s) for the application for reconsideration.
