Ohio Admin. Code 4501:1-7-01 - Application for specialty license plates issued to volunteer firefighters

(A) Any
person who is volunteer firefighter may apply for registration of one motor
vehicle and for a set of volunteer firefighter special
specialty
license plates.
(B) In order to
receive a volunteer firefighter specialized
specialty
license plate, a person shall complete BMV Form
form 4705
"Request for Special
Specialty License Plates."
(C) An accompanying document shall contain
the signatures of both the applicant and the chief or assistant chief of the
fire department certifying that the applicant is a volunteer firefighter. The
registrar may require any other evidence the registrar deems necessary to
determine whether the applicant meets all requirements for the registration of
the vehicle and the issuance of volunteer firefighter
special
specialty plates. The registrar shall reject any
application that fails to meet any requirement.
(D) The registrar shall specify the
design of the volunteer firefighter special plates in accordance with statutory
requirements.
