(1) An APRN
whose license has expired more than 30 days from the timeframe outlined in ORS
678.101 are subject to the
reactivation process. An APRN must have an active unencumbered RN license prior
to their APRN license being reactivated. An applicant must:
(a) Submit a completed application as defined
in OAR
851-006-0030(17)
for APRN re-activation;
(b) Submit
a fingerprint-based national criminal background check as described in OAR
851-001-0115;
(c) Verify their practice meets the criteria
of at least 400 hours within the two years from the date of application
submission;
(d) Has maintained
active national certification:
(A) For NPs
and CRNAs, active national certification in each area of advanced practice
congruent with license type and population foci per OAR
851-053- 0005(1)(g) is
required for licensure.
(B) For
those NPs initially licensed in Oregon prior to January 1, 2011, national
certification is not required for renewal, instead the Board requires a minimum
of 45 hours of continuing education in the population foci.
(C) National certification is not required
for CNS licensure.
(e)
Meet any other licensing criteria as defined by the Board.
(2) If the applicant cannot attest to the
above criteria, the applicant must apply for re-entry as an APRN.
(3) A licensee who has practiced in Oregon
with a license that has been expired more than 30 days must also be subject to
a civil penalty for practicing nursing in Oregon without a license.
