If an applicant cannot verify 400 hours of practice in the
preceding two years at the level of licensure sought, in Oregon or in any other
U.S. state, jurisdiction, or foreign country then an applicant must apply for
re-entry into practice.
(1) The
applicant must submit the completed application as defined in OAR
851-006-0030(17).
(2) The applicant
must hold an active, unencumbered Oregon RN license. If applicable, the RN
reentry process must be completed prior to beginning the APRN re-entry process
and must not be done simultaneously. Re-entry into APRN practice is a
self-defined Board approved process wholly dependent on the applicant for
implementation.
(3) APRN re-entry
applicants will submit evidence of:
(a) For
CNSs without national certification 100 hours of structured continuing
education completed prior to being awarded their APRN license. Continuing
Education (CE) hours must include an advanced pharmacology course meeting the
criteria in OAR
851-053-0005(3). Remaining hours of continuing education shall
consist of physical assessment, treatment modalities, patient management and
laboratory/diagnostic studies with content related to the licensure
sought.
(b) For an NP and CRNA,
active national certification in each area of advanced practice congruent with
license type and population foci per OAR
851-053-0005(1)(g) is required for
licensure.
(4)
Documentation of all CE activities must be submitted to Board staff for
approval prior to an APRN license being issued.
(5) The Board is the final approval authority
for re-entry documentation.
