Or. Admin. Code § 806-010-0040 - Architect Certificates

(1) All decorative wall certificates issued by the Board shall be signed by the Chair or the Vice Chair and the Executive Director and shall include an impression of the Seal of the Board.

(2) If a previously issued decorative wall certificate or certificate of registration has been lost or destroyed, a duplicate may be obtained by submitting a complete Application for a Duplicate Registration Request form to the Board and paying the fee under OAR 806-010-0105(a).

(3) A certificate of registration shall include the name of the registrant, mailing address, registration number, and expiration date.
