Or. Admin. Code § 918-674-0260 - Third-Party Certification Renewal
(1) Oregon certified third-party agencies
shall submit a request for certification renewal by April 1, 2000, and every
three years thereafter.
(2) The
renewal request shall be submitted on forms supplied by the Division along with
the appropriate fees.
