(a) The fee schedule for licensure as an occupational therapist shall be as follows:

Application for license...........................................	$30
Biennial renewal of license...................................	$55
Temporary license................................................	$20
Verification of licensure......................................	$15
Certification of license, scores or hours.....................	$25
(b) The fee schedule for licensure as an occupational therapy assistant shall be as follows:
Application for license...........................................	$30
Biennial renewal of license...................................	$45
Temporary license................................................	$20
Verification of licensure.......................................	$15
Certification of license, scores or hours.............	$25
(c) The fee schedule for continued competency providers and courses shall be as follows:
Initial provider approval ....................................	$40
Biennial renewal of provider approval................	$40
Individual activity approval..................................	$40
