S.D. Admin. R. 44:05:02:20 - Ambulance trip report forms
Each ground response by a licensed ambulance service must be recorded on a patient care report form approved by the department. Completed patient care report forms shall be submitted to the department on a e monthly basis. All licensed ambulance services shall keep either an electronic or paper copy of each patient care report on file for a minimum of seven years, or in the case of a minor, until they are 21 years of age.
