The following fees are nonrefundable and apply to all applicants and licensees.
| (1) | Application Fee | $200.00 |
| (2) | Biennial Renewal Fee | $120.00 |
| (3) | State Regulatory Fee - To be paid upon application and annually thereafter to be collected at biennial renewal from all license holders. | $ 5.00 ($10.00 biennially) |
| (4) | Late Renewal Fee | $ 50.00 |
| (5) | Replacement License Fee | $ 25.00 |
