Tenn. Comp. R. & Regs. 1730-05-.04 - PROCEDURES FOR CERTIFICATION
(1) The person
shall obtain an application from the Board's Administrative Office, and respond
truthfully and completely to every question or request for
information.
(2) The applicant
shall submit with the application, the non-refundable application fee, and
state regulatory fee as provided in Rule 1730-05-.06; and
(3) A passport size picture which is signed
by the applicant; and
(4) A
certified copy of the certificate of completion, or an original certificate
sent directly from the course provider to the Board's Administrative Office,
showing completion of a Board approved course on animal euthanasia.
(5) An applicant shall disclose the
circumstances surrounding any of the following:
(a) Conviction of any criminal law violation
of any country, state, or municipality, except minor traffic
violations.
(b) The denial of
certification by any other state and any disciplinary action in any
state.
(c) Loss or restriction of
certification, licensure privileges, state or federal accreditation.
(d) Any final or settled legal action that
relates to the applicant's professional services in any profession, or, any
pending legal action that relates to the applicant's professional services and
to which the applicant is a party.
(6) An applicant shall cause to be submitted
to the Board's administrative office directly from the vendor identified in the
Board's certification application materials, the result of a criminal
background check.
(7) Application
review and certification shall be governed by Rule 1730-05-.05.
