22 Tex. Admin. Code § 135.3 - Application for a Certificate of Registration
(a) The board may issue a certificate of
registration only to applicant firms that have submitted sufficient information
to meet the requirements set forth in §1001.405 of the Act and this
section.
(b) The authorized
official of the firm shall complete the form furnished by the board including
but not limited to the following information listed in paragraphs (1) - (7) of
this subsection:
(1) the name, address, and
communication number of the firm offering to engage or engaging in the practice
of professional engineering for the public in Texas;
(2) the name, position, address, and
telephone numbers of each officer or director;
(3) the name, address, and current active
Texas professional engineer license number of each engineer employee performing
engineering for the public in Texas on behalf of the firm;
(4) the name, location, telephone numbers and
the name of the engineer in responsible charge of the professional engineering
work for projects in Texas of each subsidiary or branch office offering to
engage or engaging in the practice of professional engineering for the public
in Texas, if any;
(5) the federal
employer identification number (EIN) for the firm (unless the firm is a sole
practitioner);
(6) a signed
statement attesting to the correctness and completeness of the application;
and
(7) a registration fee as
established by the board.
(c) The application fee will not be
refunded.
(d) In accordance with
§1001.405(a) of the Engineering Act, a governmental entity, as defined in
Government Code §
2254.002(1),
including a state agency as defined in Government Code §
2052.002(e),
is not subject to the firm registration requirements of this chapter.
