22 Tex. Admin. Code § 315.8 - Pharmacy Responsibility - Modification of Prescription
The pharmacy is responsible for documenting the following information regarding a modified prescription:
(1) date the change or adding of information
was authorized;
(2) information
that was authorized to be added or changed;
(3) name of the prescribing practitioner
granting the authorization; and
(4)
initials or identification code of the pharmacist.
