26 Tex. Admin. Code § 503.3 - Licensing Fees
(a) The schedule of
fees for a license is as follows:
(1) initial
license fee is $2,000;
(2) renewal
license fee is $2,000; and
(3)
change of ownership license fee is $2,000.
(b) The department will not consider an
application as officially submitted until the applicant pays the licensing fee.
The fee must accompany the application form.
(c) A license fee paid to the department is
not refundable.
(d) Any remittance
submitted to the department in payment of a required license fee must be in the
form of a personal check, certified check, or money order made payable to the
Department of State Health Services.
(e) For all renewal licenses, the department
is authorized to collect subscription and convenience fees, in amounts
determined by the TexasOnline Authority, to recover costs associated with
renewal processing through TexasOnline, in accordance with Texas Government
Code, §
2054.111.
(f) The department may make periodic reviews
of its license fee schedule to ensure that the fees imposed are in amounts
reasonable and necessary to defray the cost to the department of administering
the Act.
