Utah Admin. Code R317-15-8 - Transfer
8.1. The applicant shall give written notice
to the director of any transfer of the Certification, within 30 days after the
transfer.
8.2. The notice shall
include a written agreement between the existing and new applicant establishing
a specific date for transfer of Certification responsibility, coverage and
liability.
